A Community Transition Liaison (CTLP) identifies and reaches out to interested residents in skilled nursing facilities to encourage and coordinate discharge to the community with services by attending discharge planning meetings, connecting residents to state programs and local community support, as well as, work with resident advocate to resolve any concerns related to transitioning to the community.
