Answering 337 calls for INFORMATION and REFERRAL.

Providing 1,578 units of Information and Assistance to 283 elders.

Providing 87 HILLTOWN elders with 726 rides.

Providing DIRECT LEGAL REPRESENTATION to 3 persons (i.e. new cases.)

IN-HOME:

We increased elders' in-home options by:

Providing CARE MANAGEMENT service to 1,439 people.

Providing 606 persons with 7,193 hours of HOMEMAKER service.

Providing 260 persons with 2,293 hours of PERSONAL CARE service.

Providing 18 elders with 143 hours of CHORE service.

Providing 4 elders with 14 days of SOCIAL DAY CARE service.

Providing the caregivers of 60 elders with RESPITE CARE services.

Providing 1,133 persons with 5,002 HOME DELIVERED MEALS.

Visiting 41 elders in the community for a total of 280 hours through the SENIOR

COMPANION program and 154 units to 300 elders.

Reviewing and investigating 39 elder abuse intakes which resulted in 40 cases in

investigation with 40 PROTECTIVE SERVICE open cases and 9 cases closed during

the period.

COMMUNITY:

We increased elders' community options by:

Providing opportunities for 423 elders to get together over 3,167 MEALS at our 17

DINING CENTERS for an average of 144 meals per day.

Currently, 2 elders in the CARING HOMES program are receiving care by family

caregivers.

Paying 54 visits to elders in 12 nursing homes and rest homes through the

OMBUDSMAN program.

Receiving 82 COORDINATION OF CARE requests. Providing 74 nursing home

screenings during this period.

Assisted 25 elders with MONEY MANAGEMENT. Made 40 visits with elders.

Providing 56 units of Outreach activities to 168 elders.

Providing 171 units of Health Promotion to 42 elders.

Providing 41 units of Home Repair to 22 elders.

Providing 4 units of Vision Rehabilitation to 1 elder.

Providing 51 units of Volunteer Development to 71 elders.
Providing 85 units of Supportive Housing services to 85 elders.
Providing 51 units of Other Supportive Services to 6 caregivers.
